You do not need to be referred by a physician or other healthcare professional to join the PACE program.
- Are you aged 55yrs or older?
- Do you live within the PACE service area?
- Can you live safely in the community with support services?
- Do you have health problems that qualify for a nursing home level of care?

The enrollment process is very simple and includes an in-home assessment and interview where we explain the process and address any concerns you may have.
Our highly trained staff will help determine what your monthly premiums will be, if any, depending on your Medicare and Medicaid eligibility.
Should you then decide to enroll there will be a number of forms to complete. We will guide you through this process to make it as comfortable for you and your caregivers as possible.
All potential participants must be reviewed and approved by the Department of Health and Human Services to participate in PACE of the Southern Piedmont.
