What do we provide?

provideUpon enrollment PACE of the Southern Piedmont participants agree to receive all of their health care services through the program. We provide or coordinate everything you need to continue living safely in the home and community you love. The enrollment process is very simple. It includes an in-home assessment with you and your family or caregiver. You will learn what costs (if any) will be, and we will formulate a plan of care to improve your quality of life and support your loved ones. While most care is delivered during daylight hours, care is available 24 hours a day, 365 days a year.

The programs all-inclusive care services include, but are not limited to:

  • Primary medical and nursing care
  • Recreational therapy
  • Meals and nutritional counseling
  • Social Services
  • Pharmaceuticals and medical equipment
  • Laboratory services and diagnostics
  • Physical, Occupational and Speech therapy
  • Skilled home care
  • Hospitalization if needed
  • Skilled nursing facility care if  necessary
  • End of life care
  • Respite care
  • Socialization
  • Transport services
  • Personal care
  • Podiatry
  • Optometry
  • Audiology
  • Dental care

PACE can provide transportation to the center and to other medical appointments and activities.  In addition to all of your medical needs, we provide activities such as Bingo, reading groups, arts and crafts. There are exercise sessions, music and field trips as well.
